Through Person Centred Reviews we help each individual be involved in planning their support using tools and methods best suited to them, encouraging involvement of family, friends, advocates and professionals to look at the whole of their life; dreams, aspirations and opportunities for development along with the outcomes identified in their Care Plan provided by the Commissioning Authority. We then use the Service Delivery Action Plan (England) or Personal Plan Care (Wales) to help the individual prioritise actions they, staff, family and professionals require to meet outcomes in a way they understand and link these to a framework of skill development and increased independence, providing a balance of what’s important for the person to maintain their safety and wellbeing and important to them to make their days positive and fulfilled. The action plan is used as a signpost to person centred tools and recording systems which will be planned to help the person with each of their desired outcomes. |
